Most of this is self explanatory but we just wanted to remind you of a few things when filling this form out.
First: Fill this out completely. It helps us contact you if there are any changes.
Second: Speaking of changes, they do happen. Especially at the last minute so we need at least one of your contacts to be someone that will be at the tournament that day. If you want to list the first two contacts as club officers etc, at least put an additional contacts information in the "comments" section. Valid email addresses too please.
Third: Your registration isn't complete until both entry form and full payment are received.
Fourth: Payment is accepted through paypal only. You may pay by credit/debit card, paypal account or eCheck. This is at no additional cost to you. No payments will be accepted by mail or on the day of the tournament.
